It is our objective and philosophy that all our patients receive the best possible care and service. Patients frequently have questions regarding financial issues relating to treatment. We thought it would be helpful to inform you of Personal Endodontic, P.C.’s financial policies.

We participate with various dental insurance companies. We electronically submit all claims the day of service and day of endodontic completion. Co-payments, deductibles, and payment for services not covered by insurance are your responsibility and expected when you arrive for your appointment.

If Personal Endodontics, P.C. does not have an agreement with your insurance carrier, our insurance department will bill the insurance company as a courtesy to you. However, you are still responsible for the full payment at the time services are rendered.

Any patient over the age of 18, or an emancipated minor, will be held financially responsible for all charges incurred. For minors, the parent who accompanies the minor for their first visit will be financially responsible for the child’s care. Patients will be billed in full for any services that the health plan considers as “not a benefit” or a “non-covered service.” When and if this occurs, we will send a one-time statement for payment in full within 30 days.

Personal Endodontics, P.C. accepts cash, Visa, MasterCard, and American Express as payment for services rendered. We also participate with CareCredit for those who qualify.

Personal Endodontics, P.C. reserves the right to turn any account over to collections if it is deemed that the account has been in default of payment or compliance with this policy. Fees incurred by the collection agency will be the responsibliity of the patient (or guardian in cases of minors).

In the event you are unable to make your scheduled appointment, please provide notice at least 24 hours prior to the appointment. Personal Endodontics, P.C. reserves the right to bill our standard office visit fee for non-compliance.
